Shift : Monday – Friday with some Weekends.
Location : This position will primarily do visits in our "Team Blue" area which primarily covers the following areas and surrounding areas in the greater Apopka, Longwood, Lake Mary, Mt. Dora and Sanford area which focuses on but not limited to the following zip codes: 32757, 32777, 32798, 32712, 32768, 32703, 32779, 32746, 32771.
The role you’ll contribute:
The Registered Nurse (RN) Admission provides comprehensive skilled care as ordered by the attending physician. The RN educates patients and their family members and ensures the safety of the patient. The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. We adopt the RN scope of practice as outlined by the state for its licensed RN personnel. The RN is responsible for any and all practices and duties within the scope of practice as outlined by the state.

· Provides skilled nursing care in a patient's home as ordered by the attending physician.
· Observes and monitors patient conditions.
· Performs OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
· Administers medication as prescribed by the physician.
· Helps decrease re-hospitalizations by front loading visits for high risk patients.
· Manages multi-disciplinary care as applicable.
Qualifications
The expertise and experiences you’ll need to succeed:
· Associate's degree in nursing.
· Minimum of one year experience as a Registered Nurse in medical/surgical, home health experience
· Basic Life Support - CPR Cert
· Driver's License
· Registered Nurse Licensure
· A clear/active Florida Registered Nurse (RN) license
· Maintain a current, valid Florida Driver's License in good standing with a safe driving record and a reliable vehicle.
· Maintain valid personal automobile insurance.
