The role you’ll contribute:
Participates in patient assessment, counseling, support, and resource linkage. Provides counseling to patients and families as well as organizing and leading support groups. Works closely with physicians and other medical staff to aid patients in understanding treatment options, facilitating decision making, as well as providing a variety of other case work services in acute care areas, outpatient, and in the community.

Assesses patient’s psychosocial history, cultural and spiritual needs through observation, interviews, and clinical data.
Identifies patient needs and current resources (e.g., financial, patient rights, insurance, family support).
Provides social casework services as part of an interdisciplinary approach.
Participates in rounds, case conferences and committees to facilitate appropriate patient services.
Qualifications
The expertise and experiences you’ll need to succeed:
Master’s Degree in Social Work, Counseling or related
One year of related clinical experience
Demonstrates clinical and ethical professionalism, conflict resolution and critical thinking skills
Current State License as a Clinical Social Worker (CSW), Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC)
Current Basic Life Support from the American Heart Association
