Work Hours/Shift:
Full Time
You Will Be Responsible For:
  • Evaluates all referrals and identifies discharge planning needs in accordance with established criteria.
  • Communicates with the patient/family and healthcare team to assess and identify individual discharge needs and desires, including those specific to the age of the patient being served (neonate, child, adult, and geriatric).
  • Assists the patient/family and healthcare team in implementing the discharge plan.
  • Assists the patient/family in investigating eligibility and applying for Medicaid or SSI through Meds by referring to the financial counselor.
  • Utilizes community resources and refers the patient /family to appropriate agencies and services when necessary.
  • Communicates with extended care facilities to assure bed availability and arranges placement.

What You Will Need:
  • 2 years of experience in Social Work in a health care setting.
  • Bachelor’s Degree in Social Work
  • 1 hour Hip and Knee Joint replacement education within 6 months of hire date, then annually
  • 2 hours Stroke education within 6 months of hire date, then 1 hour annually
  • Masters in Social Work (preferred)
  • State of FL Social Worker license (preferred)
Job Summary:

Assists the patient, family/guardian and all members of the healthcare team in the discharge planning process. Contributes to the monitoring of the quality and appropriateness of patient care delivered. Provides for safe and supportive environment for all patients, families and coworkers. Promotes and provides for optimum patient outcomes through use of Social Service concepts. Demonstrate positive communication and interpersonal skills with patients, families and coworkers.
