Work Hours/Shift:

  • Full-Time, Monday – Friday

You Will Be Responsible For:

  • Abstracts pertinent information from patient documentation/medical record and instructions provided by referring entities in order to assign accurate ICD-10 codes following appropriate coding guidelines
  • Seeks additional information as needed from the agency/physician to ensure complete information
  • Enters information into Electronic Medical Record (EMR) system(s)
  • Communicates with agency/physician when code assignments are not straightforward or documentation in the record is inadequate, ambiguous, or unclear for coding purposes
  • Keeps abreast of coding guidelines and reimbursement reporting requirements
  • Follows the most recent ICD-10 coding guidelines and conventions.
  • Maintains quality of coding review with score of 95% or greater on ongoing quality audits.
  • Maintains professional and technical knowledge, including required certifications and continuing education credits by attending educational workshops and reviewing professional publications
  • Attends/participates in ongoing learning and education in ICD-10 coding, and industry and regulatory changes.
  • Completes documentation reviews at end of episode to determine compliance with regulatory requirements as directed.
  • Provides follow up related to bill hold items to facilitate timely resolution of issues
  • Develop, maintain knowledge and proficiency in use of software systems utilized by organization
  • Excellent verbal and written communication skills, and ability to relate well in a remote environment under the supervision of the Medical Review Manager
  • Adheres to federal/state guidelines, rules and regulations, statutes and law.
  • Utilizes time management skills to achieve daily productivity standards and documentation submission requirements.
  • Flexibility as may be asked to work on additional projects
Qualifications

What You Will Need:

  • At least 1 year of experience in diagnosis coding of home care records
  • Minimum of 1-year experience in a computerized office environment and knowledge/familiarity with home health
  • Home care agency work experience, preferred
  • Minimum of 1-year experience in a computerized office environment and knowledge/familiarity with home health, preferred
  • At least 1 year of ICD-10 coding experience in Home Care, preferred
  • Familiarity with OASIS / certification related to OASIS, preferred
  • ICD-10 Home Care coding certification; the following are recognized certifications:
    • Certified Professional Coder (CPC)
    • Certified Coding Specialist (CCS)
    • Home Care Coding Specialist (HCS-D)
    • Board Certified Home Health Coder (BCHH-C)
    • Registered Health Information Technician (RHIT)
    • Registered Health Information Administrator (RHIA)

Job Summary:

AdventHealth Central Billing Office (CBO) provides coding and medical review services for system-wide home care and hospice agencies. The Home-Based Services (HBS) Coding / Chart Review Specialist is responsible for reviewing and analyzing documentation present in the medical record to assign diagnoses codes as described by the physician(s) of record. Ensures appropriate ICD-10CM coding and sequencing and works with agency staff to clarify and correct documentation and data integrity issues as they relate to coding, OASIS and the plan of care.  May also perform medical review of records for presence of documentation supporting regulatory requirements.

The employee must be able to function with a high degree of independence and considerable discretion as work may completed onsite or remotely.
