You Will Be Responsible For:   

  • Social Services
    • Performs all Master’s-level social work psychosocial activities. Upholds professional standards by adhering to the National Association of Social Workers (NASW) code of ethics. Provides illness-adjustment counseling in the inpatient and outpatient settings including supportive, adjustment, and crisis intervention. Provides crisis intervention for high-risk situations.
  • Care Coordination
    • Coordinates discharge planning from hospital to home and conducts home visits to ensure the client is safe, healthy and thriving in their environment.
  • Counseling and Education
    • Participates in the development of programs for Alzheimer’s and Dementia patients, families and caregivers.
  • Advocacy
    • Provides patient/caregiver with an Advance Directive form and explains how to correctly complete form.
  • Other
    • Participates in continuing and appropriate learning experiences, both internally and externally. Attends pertinent conferences education and seminars regarding clinical social work (subjects pertinent to aging population).


Qualifications
What Will You Need:        
 

EDUCATION AND EXPERIENCE REQUIRED:

  • None
 

EDUCATION AND EXPERIENCE PREFERRED:

  • Three years of experience in providing psychotherapy
  • Exposure to Geriatric  population in clinical practice
  • Three years of post-Master’s experience (in clinical practice preferred)
  • One year of experience with field supervision of Master’s-level student interns
  • One year of experience working in short-term/solution-oriented modalities and group
  • Home health experience
  • Experience in Montreal Cognitive Assessment (MoCA), dementia screening and caretaker burden
 

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

  • Current active State of Florida license as a Licensed Clinical Social Worker (LCSW)
 

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:

  • Certified Case Manager (CCM) issued by the Commission for Case Manager Certification (CCMC) or the Academy of Certified Case Managers (ACCM)
 
 
Job Summary:

The Licensed Clinical Social Worker provides social work intervention to Alzheimer’s and Dementia patients and their families/caregivers throughout the treatment continuum in the form of supportive/illness adjustment counseling and resource/concrete service assistance in an effort to minimize the psychosocial aspects of illness. Assesses patients in the outpatient, inpatient, private home or other senior living community for psychosocial and economic barriers that interfere with the attainment of healthcare goals. Works in a multidisciplinary, collaborative team effort with physicians, nursing staff, case managers, spiritual support staff and community resources in determining patient’s medical status, treatment plan and prognosis as they relate to and affect psychosocial care, long-term care and end-of-life planning.  Advocates for patients and families with counseling services, crisis intervention, stress management, patient and family coping, bereavement, abuse and neglect issues and continuity of care.  Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
