Work Hours/Shift:

Part Time / Days

You Will Be Responsible For:

  • Follows established procedures for chart audit and abstraction, entry to databases and adherence to data deadlines
  • Coordinates abstraction schedule and priorities with Quality RN
  • Extracts electronic medical record data on site
  • Enters pertinent data from paper and electronic medical records into multiple databases as required by established abstraction procedures
  • Communicates effectively and professionally with healthcare team members throughout the facility when variance or opportunities for improvement are identified
  • Meets or exceeds interrater reliability audits
  • Assists Quality RN with on-site rounding and education to facilitate increased compliance of documentation from healthcare team members
  • Reviews admissions within the last 24 hours (72 hours on Mondays) for clinical audits and processes
  • Assists to review interdepartmental chart review to facilitate accurate billing and quality metrics. (HIM record review requests for patient safety or diagnosis verification)
  • Assists team to facilitate environmental audits throughout the facility to ensure regulatory compliance
  • Completes all abstractions queued monthly
  • Attends webinars/conference calls as directed by Quality RN
  • Completes facility education requirements
  • Verifies all abstraction submissions for CMS/JC are competed and correct quarterly

Qualifications

What You Will Need:

  • High school diploma
  • Minimum two years’ relevant experience involving medical record documentation or medical claims experience
  • Certification in Medical Abstraction preferred
  • CNA, LVN, RN or similar medical experience preferred
  • RHIA certification preferred
  • CPHQ certification preferred
  • HCQM certification preferred
  • Must perform abstractions with a high degree of accuracy
  • Basic knowledge of medical language and patient care to facilitate interpretation of data and its application to the criteria for which data is being abstracted
  • Effective computer communication skills
  • Strong organizational skills, with the ability to multi-task and prioritize based on deadlines, criticality and directives provided
  • Detailed knowledge of computers/systems including Word, Excel, Power Point and Outlook
  • Ability to interpret guidelines and apply criteria independently to ensure data quality

Job Summary:

Screens, abstracts and assists the analytical process of medical records against quality, safety and service measures that comply with state, federal and regulatory quality initiatives. Demonstrates the ability to collect data with highest levels of accuracy and timeliness.  Ability to meet data entry deadlines.  Actively participates in collaborating and educating nursing and medical staff to achieve improvement in all initiatives as directed by Quality RN.
