You Will Be Responsible For:

  • Serves as a primary contact and advocates for patients throughout the continuum of care (prevention, diagnosis, treatment, survivorship, and end of life).
  • Maintains accountability and responsibility for access, facilitation, and coordination of new patient visits.
  • Ensures a smooth transition and coordination of care for patients being referred from the Breast Imaging RN Navigator.
  • Collaborates with the multidisciplinary team to address patient and family concerns, problems and needs including medical, psychological, financial, spiritual, emotional, and socioeconomic.
  • Identifies barriers to care and makes appropriate referrals to external and internal resources.
  • Functions as a liaison with referring services.
  • Leads the multidisciplinary treatment care plan implementation and evaluation.
  • Provides patient and family with appropriate education and informational materials regarding diagnosis, disease, treatment, and symptom management.
  • Educates eligible patients about appropriate clinical research studies and technologies.
  • Serves as a liaison between breast imaging, medical oncology, radiation therapy, and surgical teams.
  • Collaborates with the patient, family, ancillary departments, and community resources to achieve the goal of a multidisciplinary health care team who provides care that is well coordinated and consistent across the continuum of care.
  • Develops and delivers an individualized patient Cancer Survivorship Care Plan in coordination with Providers.
  • Provides cross coverage for the Breast Imaging RN Navigator as needed.
  • Develops and maintains a resource library of breast health educational materials for patients that is language specific.
  • Coordinates and provides logistical support for educational and community outreach events.
  • Documents throughout the patient care continuum in the database or identified software solution.
  • Leads breast cancer support groups and attends the weekly Breast Tumor Boards.
  • Serves as a member of the Breast Care Leadership Committee and assists in achieving accreditation standard compliance.
  • Participates in oncology business development through involvement with local and regional outreach efforts and professional relationship building with referring and specialty members of the healthcare community.
  • Identifies quality improvement initiatives and actively participates with the Quality Improvement (QI) team to design, conduct and measure response to QI initiatives.

  • Database management knowledge

  • Graduate of ASN Program upon hire
  • Minimum of 3 years healthcare experience upon hire


  • Medical Oncology, Diagnostic Imaging, Breast Clinic, or Radiation Oncology clinical experience
  • Healthcare marketing and business development work experience
  • BSN


  • Valid FL Driver's license upon hire
  • Current American Heart Association BLS certification or RQI upon hire
  • Current valid State of Florida Board of Nursing license upon hire


  • Oncology specialty certification preferred (i.e. Oncology Certified Nurse, Oncology Nurse Navigator, Oncology Patient Navigator or Certified Breast Care Nurse).

Job Summary:

The Breast Oncology Nurse Navigator serves as a patient liaison to help patients and family members navigate the complex healthcare system. The Breast Oncology Nurse Navigator will establish early contact to help identify and eliminate barriers to timely diagnosis, treatment, and survivorship care. The Breast Oncology Nurse Navigator will serve as a resource for breast cancer patients throughout the continuum of care and provides ongoing assessment of patient and caregiver needs. The Breast Oncology Nurse Navigator ensures patients are effectively and efficiently navigated through the health care system receiving appropriate referrals, support, and educational services through their journey.  This position works with a multidisciplinary team to provide improved comprehensive oncology services to breast cancer patients.
