RN Transitional Care Specialist-  Texas Health Mansfield- operated as joint venture between Texas Health Resources and AdventHealth

Location: 2300 Lone Star Road; Mansfield, TX

Work Hours/Shift:

Full Time / Days

You Will Be Responsible For:

•    Coordinates care of patients at risk for readmission from admission through 30 days post-discharge.

•    Executes readmission prevention strategies.

•    Enables and manages readmission prevention measures at all levels of care (hospital and community, patient and provider).

•    Reviews and analyses the readmission prevention process.

•    Arranges for post-discharge support services if needed.

•    Coordinates post-discharge phone calls.

•    Provides direct interaction with care providers such as direct care nurses, physicians, social support and utilization (social services, care management), post-acute care providers (home health, LTC, rehab, medical home).

•    Remains flexible to changing systems, is expected to demonstrated quality and effectiveness in work habits and treats staff, physicians, patients and families with consideration and respect.

•    Recommends new services that can better serve our patients in regards to preventing readmissions.

•    Provides community awareness programs to educate residents and referring agencies of services offered.

•    Effectively communicates with all levels of the hospital and medical staff.

•    Provides, coordinates and directs the activities of various levels of assigned nurses and ancillary personnel in the provision of patient care utilizing both professional and independent judgment.

•    Participates on various hospital committees and task forces as required.

•     Collaborates with other hospital departments including Medical Management & Case Management to promote quality and safety for all patients.

•    Understands the Computerized Physician Order entry process and proficiently uses the electronic medical record for retrieval of patient information.

•    Monitors Electronic Medical Record for new physician orders and diagnostic results.

What you will need:
  • A registered nurse with BSN required
  • 2-5 years nursing experience required
  • Demonstrates knowledge of community resources and medical community
    Advanced practice degree preferred
    Home health or public health nursing is helpful
  • Provides supervision and guidance to transitional nurses
