Work Hours/Shifts


What You Will be Responsible for:
  • Displays good theoretical knowledge of psychopathology and appropriate treatment techniques for each type of therapy.
  • Responsible for performing psychosocial assessments on patients to determine social service needs, diagnosis and treatment plan. The assessment is completed within 60 hours of admission.
  • Evaluates patient data, develops and implements a treatment plan for the patient.
  • Ability to communicate effectively with physician regarding social service needs of the patient. Assists the physician to facilitate post-hospital care.
  • Participates in weekly treatment planning for each patient, works cooperatively as a member of the interdisciplinary treatment team, completes progress notes and discharge plans in a timely fashion.
  • Provides group therapy. Provides individual and family therapy as appropriate.
  • Maintains current knowledge of social, medical and psychiatric services available within the community, maintains supply of resource materials to be distributed to patients when needed. Is able to obtain other resources as needed.
  • Communicates with family members and caretakers regarding the needs of the patient and anticipated plans as appropriate. 
  • Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Accurately determines type of assistance/setting/resources necessary for the patient/family and provides appropriate resources/assistance/list of facilities.
  • Participates in Transformation.
  • Dependable, arrives at work on time, reports to work as scheduled, and adheres to break and meal schedules.
  • Schedules time off in advance. Punches in and out as scheduled. Calls supervisor to report absences timely.
  • Agrees to work extra shifts/hours as needed. 
  • Completes work assignments thoroughly and follows up on requests in an accurate, prompt and timely manner.
  • When working Intake: Assesses plans, evaluates and implements admissions for all Behavioral Health units according to department specific admission criteria. Reports findings to M.D. for final disposition of each call. Reports admission data, including preliminary admission diagnosis, medication administration reconciliation data, and estimated arrival information to the Charge RN on the designated inpatient unit.
What You Will Need:
  • Demonstrate knowledge and skills to perform duties, methods and procedures required by the job.
  • Maintains strict confidence of patient and/or customer information.
  • Suggest and participate in process improvement opportunities in the performance of duties.
  • Documents patient care and completes task list in accordance to standards of care and other protocol.
  • Seeks solutions to resolve unexpected problems that arise in the course of their work, and make practical routine decisions.
  • Current CPI required within 90 days of hire then maintained
  • Additional languages preferred.
  • Master's Degree in Social Work or in related field required. 
  • Two or more years of experience in social service or related field with an emphasis in psychiatric work, preferred.
  • Current license as LCSW, LPC or LCP-A required.

Job Summary 

Responsible for providing social service functions in the behavioral health care setting. Provide assessments, treatments and case management to patients and families. This person is a resource to healthcare professionals concerning psycho-social patient needs. Coordinates discharge planning beginning on the date of admission for each patient. Provides group therapy according to programming needs, and other assignments as directed by the Department Director.
