Work Hours/Shift:

  • Full-Time, Monday – Friday

You Will Be Responsible For:

  • Conduct patient outreach to coordinate care for preventative and chronic disease management services to complete gaps in care.
  • Interacts with other departments to ensure the smooth transfer of member information across the continuum of care
  • Works closely with Provider offices either onsite or remotely to help provide support in managing the patient population including working and analyzing reports, identify opportunities for improvement, contacting patients via phone, mail or other means.
  • Assist assigned practices with managing their populations that can include, but not limited to, specific chronic diseases, high ER utilizers, medication compliance, and other identified areas of focus.
  • Monitor practice progress on an ongoing basis, which may include analysis of data and identifying opportunities for improvement.
  • Identifies problematic issues, determines reasons and actions taken to resolve, and escalates appropriately
  • Review patients’ data including clinical/claims history, outpatient treatments, medications, electronic medical records, chart reviews or other information to assist in monitoring and facilitation of approved standards of care.
  • Assist with coordination of care/services and transitions of care
  • Analyze documentation and submit appropriate metric and reporting for Quality programs.
  • Communicates clearly, regularly and effectively with all levels and functions to create an atmosphere of mutual respect, openness and encouragement.
  • Maintains the confidentiality of the meetings attended.
  • Work closely with Chief Medical Officer and Assists members of the quality department/ quality committee and practices with any special projects as needed
  • Develop educational documentation to inform providers about physician initiatives.
  • Implements monitoring programs to provide feedback to providers on physician initiatives.
  • Work with IS team to direct, teach and guide providers and practice personnel, serving as a resource person and role model to aid in their learning process in clinical information systems.
  • Ensures quality and accuracy of educational materials provided.
  • Maintains current knowledge of all changes in system functionality.
  • Serves as a liaison between users, physicians, data processing, and practice management on physician initiatives

What You Will Need:

Education and Experience Required:

  • Associates Degree
  • Knowledge of primary care practice and appointment setting process
  • Experience with evaluating and monitoring data around quality metrics

Knowledge and Skills Required:

  • Experience with Athena, NextGen or Cerner workflow preferred
  • Clear and confident communicator
  • Possess strong attention to detail
  • Ability to work independently with minimal oversight
  • Excellent time management skills
  • Must highly value integrity, honesty and confidentiality
  • Must be proficient in MS Word, Excel, and PowerPoint applications. 
  • Must have knowledge and skills necessary to deliver quality educational instruction to all levels of staff in a clear and concise manner in both written and verbal contexts.
  • Being adaptable and maintaining effectiveness when experiencing major changes in work tasks or the work environment; adjusting effectively to work within new work structures, processes or requirements
  • Ability to work independently. Being resourceful and proactive

Job Summary:

The Quality Improvement Specialist will provide population outreach and quality metrics support for Physicians and APPs within AdventHealth Medical Group. This support will include, but not limited to, monitoring of progress towards network goals, gap management, reviewing medical record data/chart reviews to close gaps in care, transitions of care for identified populations, and care coordination to drive quality and cost outcomes for the attributed populations. The Quality Improvement Specialist will be responsible for assisting with the development of process workflows, monitoring metrics/tools and policies and procedures as requested and will be responsible for adhering to the AHS Corporate Compliance Plan, to the rules and regulations of all applicable local, state, and federal agencies, and to the standards of all accrediting bodies.
