Location: UChicago Medicine AdventHealth GlenOaks Hosp – Behavioral Health, 701 Winthrop Ave, Glendale Heights, IL
The role you’ll contribute:
· Provides inpatient behavioral health case management and discharge planning services.
The value you’ll bring to the team:
· Conducts comprehensive assessments to include risk assessment and establishes treatment objectives
· Provides referrals for patients within the agency and community
· Conducts individual and family case management sessions
· Conducts group therapy on the unit
· Documents treatment observations and maintains records, while adhering to confidentiality protocols
· Evaluates patient progress and treatment intervention effectiveness
· Provides patients information about community resources and therapeutic activities
· Works directly with patients, family members, physicians, and community providers to ensure collaborative and continuous care
· Maintains solid clinical skills including knowledge of DSM-IV-TR and DSM-5, community resources, process of payor source identification, and short-term intervention strategies
· Evaluate patient data, develops and implements a treatment plan for the patient
· Responsible for performing psychosocial assessments on patients to determine social service needs, diagnosis and treatment plan. The assessment to be completed within 72 hours of admission
· Identify and intervene for suspected abuse, including sensitivity to age specific concerns
· Participate in the multidisciplinary treatment team meetings, providing clinical input
· Formulates discharge plan in collaboration with the multidisciplinary treatment team, the patient and the family. Assure outpatient appointments are scheduled upon inpatient discharge
· Coordinates the continuum of care of patients between Behavioral Health Programs
· Completes Certificate for Involuntary Admission when necessary
· Demonstrate working knowledge of the involuntary commitment process
· Opt-In participation in Emergency Notification System as applicable to facility
· Performs other duties as assigned
SUPERVISORY RESPONSIBILITIES
· Provides supervision to LSW/LPC associates when necessary
· Oversees distribution of caseload to case management team
· Conducts chart audits to ensure documentation compliance
· Provides necessary updates to quality and safety committee and other leadership committees as necessary
· Participates in performance improvement action planning and implementation
Qualifications
The expertise and experiences you’ll need to succeed:
EDUCATION AND EXPERIENCE
· Master's degree in social work, psychology, or related field required
· Two years of relevant experience required
· One year of experience in a healthcare setting preferred
· Two plus years of supervisor experience preferred
LICENSURE, CERTIFICATION OR REGISTRATION
· Formal De-escalation Training – must be obtained within 90 days of employment
· Independently licensed in the State of Illinois (LCSW, LCPC) required upon hire or transfer date
KNOWLEDGE AND EXPERIENCE STRONGLY PREFERRED
· Ability to gather individual patient assessment data, including individualized treatment plans, and referrals
· Ability to analyze and assess data, techniques, methodology, equipment operations, and quality control to ensure that information is obtained and presented accurately
· Ability to master educational, counseling, and administrative principles, theories, techniques, and practices
· Ability to work collaboratively as a dynamic treatment team member
· Basic computer skills and knowledge of Microsoft Office
· Must be able to read, write and speak conversational English
· Suggest and participate in process improvement opportunities in the performance of duties
