Work Hours/Shift:  
  • Full Time / Day
You Will Be Responsible For:
  • Provides medical care for hospice patients.  
  • Utilizes assessment, intervention and teaching to maximize patient comfort and to enhance the quality of life to the patient/family unit.
  • Evaluates the health status of selected patients, orders treatment plans and provides education to patients and their families under the supervision of a licensed physician.
  • Performs clinical evaluations and reviews diagnostic studies of patients under the supervision of a licensed physician.
  • Participates in interdisciplinary team conferences of the facility teams, as needed.
  • Interacts with patients and families to manage the psychological, social and family issues regarding hospice and the services rendered.
Qualifications

A.     Education and Training: Master’s Degree in Nursing with Nurse Practitioner designation is required.

B.     Experience: A minimum of 3 years of experience in medical nursing, hospice and/or critical care experience and a minimum of 2 years experience as an APRN is required.  Experience with symptom and pain management is preferred.

C.     Certificates and Licenses Required: Clear/Active Florida APRN license required.  Basic Life Support certification required. Certified Hospice and Palliative Nursing Certification is preferred. 


Job Summary:

The APRN is responsible for evaluating and coordinating patient/family needs to enable preventative and supportive care under supervision of the Executive Medical Director.
