You Will Be Responsible For:
Handle all patient admission & discharge aspects.
Prepare, complete and register admission.
Manage and solve admission and discharge problem in collaboration with admission department, CM, TL and all other interdisciplinary team members.
Initiate and carry out nursing care plans (IPOC).
Initiates facility core measure and ensure to discuss during handoff communication.
Collect patient medical history.
Establish and maintain positive relationship with patients and their families.
Provide family member information about zone tools, patient condition, plan of care
Follow hospital policies and procedures and make ethical decisions based upon consent and confidentiality.
Collaborate with nursing staff and attending physicians in establishing patient admission and discharge process.
Meet patient needs during the admission and discharge process.
Provide high nursing services quality during the admission and discharge process.
Ensure compliance with health care admission policy and regulations.
Ensure safe and clean environment for patients.
Adheres to organizational standards for dress and professional appearance
Demonstrates collaboration and teamwork in managing patient flow-i.e. promotes timely patient placement by following hospital policy for patient admission, discharge and transfer.
Participates in peer interviewing and peer review process.
Considers factors related to safety, effectiveness, cost and impact on practice in the delivery of nursing services
Implementation: Implements the identified plans for patients and unit specific process improvement with appropriate nursing interventions and physicians orders in an accurate, timely manner and documents intervention according to hospital policy. Documentation meets criteria for core measure, charges, PI improvement, and all quality/regulatory standards as measured by chart audits. Coordinates implementation of the plan of care through leadership and delegation appropriate to skill level including but not limited to: LPNs, NTs, CNAs, Unit Secretaries, RTs, PTs. Educates patient/family based on assessed learning needs.
What You Will Need:
Valid American Heart Association (AHA) Basic Life Support (BLS) certification
Valid AHA Advanced life support certification (ACLS) per specialty/unit requirements
Graduate of an accredited school of Nursing, minimum of associates degree as Registered Nurse
Job Summary:
The admission & discharge Registered Nurse ensures the smooth entry of patients into the care continuum by enabling all necessary preparations for a patient care including obtaining all necessary documents related to the admission and discharge process. The admission & discharge nurse works with other members of the medical care team in coordinating the admission process for the patient. Discharge planning process begin at admission. The nurse care plan is updated and resolved throughout the patient length of stay. At discharge, nursing problems are either resolved or progress toward resolution and follow plans are noted. The patient and family are taught about the diagnosis problems, illness and/or surgery; they have an opportunity to practice procedures and to learn about dressing changes, tube and drains, trach care, medication and nutrition. The admission & discharge nurse is directed by the Chief Nursing Officer.
