You Will Be Responsible For:

  • Screens patients presenting to the Emergency Department (ED) with complaints related to mental health and/or substance abuse issues.
  • Provides referral resources to patients in the Emergency Department who do not meet criteria for admission to the Behavioral Health Unit (BHU).
  • Assists and arranges discharge to other behavioral health facilities for patients who do not meet criteria for admission to the BHU, but require in-patient behavioral health treatment.
  • Coordinates discharges to Baker Act Receiving Facilities for patients in the ED and on the BHU.
  • Acts as a resource for behavioral health needs to ED staff, in-patient nursing units, patients, and family.
  • Monitors clients' condition; advises other mental health professionals of client's condition and treatment needs; reports changes in condition to physician.
  • Conducts pre-authorization from insurance company for patients admitted to the BHU.
  • Documents admission screening on patients accepted to the BHU in Cerner.
  • Educates patient and family on the Voluntary legal status and the Right to Request Discharge process.
  • Pro-actively screens patients in the ED for opportunities to convert involuntary to voluntary status under the Baker Act.  Notifies psychiatrist and/or BHU Nurse Manager of potential conversions.
  •  Participates in Tele-Mental health evaluations.  Coordinates consult with psychiatrist. Educates patient about Tele-mental health. Coordinates follow up care and transition to community care provider for patients discharged by  Tele-Mental Health consult.
  • Screens patients on the medical floors who have been referred to the BHU for possible admission.
  • Responds to “Code Gray’s” in the facility when available to act as a resource and liaison for behavioral health needs.
  • Assists on the BHU with patient care needs.  Assist with meals, patient requests, monitoring safety of patients and unit via Q 15 minute safety checks and safety checks of the environment.
  • Conducts group related to mental health and substance abuse issues and the social issues that affect such patients. 
  • Assists LCSW/In Patient Therapist with discharge planning.  Helps coordinate appointments once discharge needs have been identified by the LCSW/In Patient Therapist. Gathers data for psycho-social assessments.
  • Completes data entry for billing information related to psychiatrist’s daily rounds and consults.
  • Communicates with the patient/family and health care team to assess and identify individual plan of care needs and preferences, including those specific to the age of the patient being serviced (neonate, child, adolescent, adult, and geriatric).
  • Maintains communication with community mental health providers to facilitate access to care for patients discharged from the ED and BHU.
  • Uses verbal de-escalation skills to diffuse verbally and/or physically abusive and aggressive clients and performs aggression control techniques, if needed.
  • Handles crisis calls by communicating effectively with suicidal, mentally, emotionally, and/or psychologically disturbed clients over the telephone to diffuse potentially life-threatening situations; provides appropriate treatment/intervention and follow-up.

What You Will Need:

  • Associates Degree in Nursing along with a current and valid nursing license for the state of Florida OR Bachelors’ degree in psychology, social work, mental health counseling, or related field
  • One year of clinical mental health/substance abuse experience or related experience
  • One year experience in psychiatric setting or any equivalent combination of training and experience which provides the following knowledge, abilities, and skills:
  • Complete Aggression Control Techniques, CPR, First Aid and AIDS training during the first ninety (90) days of employment.

Job Summary:

The Behavioral Health Care Coordinator functions under the Behavioral Health Unit and reports to the Director of ED and Behavioral Health. The Behavioral Health Care Coordinator is a highly responsible and specialized direct service clinical worker functioning as a screener serving a variety of populations who have mental and substance abuse issues.  The Behavioral Health Care Coordinator works in the ED and the BHU and serves as a liaison between the two departments to coordinate admissions and care of behavioral health patients.  Work involves screening and assessing patients for appropriate psychiatric disposition, obtaining authorizations for services and treatment, and making referrals to community agencies. 

Works as an extension of the Behavioral Health Unit staff to assist with patient needs on the BHU.   Performs initial diagnosis in accordance with DSM-V criteria; performs screening for Baker Act Criteria or other established program criteria; gathers information for psycho-social assessments, mental status exams, and treatment plan; documents patients’ mental health history; presents information to admission committee/psychiatrist; makes referrals for discharge planning and transition of care. Coordinates Baker Act placement and discharges to Baker Act Receiving facilities for patients in the ED and the BHU.  Participates in the Tele-Mental Health consult process. Maintains communications with necessary outside agencies and institutions to assure the patient continuum of care beyond the primary care physician and hospital.  Utilizes effective communication techniques to promote patient, and family compliance with the anticipated plan of care.  Demonstrates competency within the scope of practice, as appropriate to the ages of the patient services (neonate, child, adolescent, adult, and geriatric) and maintains knowledge of growth and development with an understanding of the range of treatment needed by these patients. 

Work is performed in accordance with established policies, procedures, and state and federal regulations. Work is reviewed through observation, presentations, discussions, clinical supervision and case records.
