Work Hours/Shift:

Full Time / Days

You Will Be Responsible For:

  • Schedules physician follow-up appointments for patients discharged from ED/inpatient

  • Coordinates appointments for ER/inpatient patients who do not have a documented primary care physician.

  • Coordinates appointments with Specialist if the ER/inpatient physician dictates that follow-up is prescribed.

  • Contributes to the day-to-day processes of the ER unit/inpatient unit.

Qualifications

What You Will Need:

  • Three years of hospital experience
  • Effective communication
  • Effective follow-through
  • Decision making skills
  • Independent judgment

 Job Summary:

The Care Navigator will be dedicated to superior customer service and satisfaction. This position will develop and maintain positive relationships with both internal and external customers while conveying a positive image. Coordinates and directs patient care to ensure patients’ needs are met and hospital policy is followed.
