Program Director – Population Health CIN Orlando

Work Hours/Shift:

  • Full Time – 8a – 5p, M- F

You Will Be Responsible For:    

•     Divisional position that supports Population Health and CIN efforts for Central Florida Division (North and South)

•     Develops & Deploys Convenient Care that Improves Patient Access (Nurse Triage, Telehealth, Asynchronous, PCMH, Pod Structure)

•     Develops & Deploys Connected Care with focus on network integrity

•     Improves Connected Care through IT Connectivity, Ambulatory Relationships

•     Develops & Deploys Complete Care with focus on whole person health wrap around services including Behavioral & Spiritual Health

•     Oversees operationalization of quality & utilization efforts with Medical Directors

•     Ongoing Program Development that address the opportunities and data drivers to improve quality, cost, and experience.

•     Oversight responsibilities for the development, deployment and monitoring of integrated cross continuum ambulatory pharmacy services, such as medication therapy management and medication reconciliation, behavioral social health services, and health related social needs.

•     Oversight responsibilities for ensuring CREATION Health is a central function of all population health offerings.

 
COMPLIANCE/REGULATORY RESPONSIBILITY:

•     Responsible for ensuring abiding by stark laws and HIPPA

 
OPERATING & CAPITAL BUDGET/FINANCIAL RESPONSIBILITY:

•     Meets budget expectations for the Advent Health Population Health and CIN cost centers

•     Coordinates any necessary capital spending related to new space, new equipment, or spending of foundation accounts for capital needs for Population Health

•     Ensures appropriate analysis and corrective action takes place when necessary based on financial performance of contracts

STRATEGIC PLANNING RESPONSIBILITY:

•     Responsible for developing of long and short-term business goals and strategies for Population Health to ensure growth and success

•     Leads to leverage Population Health to further improve the health of our community and reduce the per-person cost of healthcare by developing and growing programs that focus on convenient, connected, and complete care.

PERFORMANCE IMPROVEMENT RESPONSIBILITY:

•     Performs ongoing review of Population Health program and educational effectiveness and leads change efforts accordingly.

•     Provides oversight and direction to the leadership team for development and implementation of performance improvement activities to help improve Population Health performance.

•     Identifies opportunities for cost savings that drives contract performance.

•     Leads Process Improvement of CIN physician led committees A3s

COMMUNITY RELATIONS RESPONSIBILITY:

•     Develops partnerships and serves as the executive liaison and consultant to Population Health related corporate and community partners.

•     Upholds strong client relationship management with employers and payors.

STAFFING/LABOR/SUPERVISORY RESPONSIBILITY:

•     Responsible for ensuring smooth operations of the population health teams and operations

•     Oversees the Population Health leadership team responsible for daily operations in compliance with financial critical success factors

•     Promotes a positive work environment that leads to high team engagement


Qualifications

EDUCATION REQUIRED:

•    Bachelor’s Degree in Business, Healthcare Administration or other relevant field of study

EXPERIENCE REQUIRED:

•    Five years’ experience in a health care related field

•    Three years’ experience in healthcare or clinical management/leadership

KNOWLEDGE AND SKILLS REQUIRED:

•    Excellent analytical and problem-solving skills

•    Effective oral and written communication skills, with the ability to articulate complex information in understandable terms to all levels of staff

•    Effective computer skills, particularly Microsoft Power Point, Office Outlook, Word, Excel

•    Ability to work in a matrix-management environment to achieve organizational goals

•    Strong interpersonal skills
