You Will Be Responsible For:
• Evaluate patients as assigned and collaborate with team members as appropriate for optimal patient care/service.
• Establish POC with goals based on patient’s current level of functioning and potential for improvement and in accordance with physician orders.
• Implement OT assessment and care plan, continuously evaluating the patient’s response to program and need for modification.
• Identify specific, achievable goals in collaboration with the patient and/or caregiver under the physician’s direction.
• Instruct patient and family in total OT program; including self-care and creative activities that are directed toward improving independence, physical and mental functioning.
• Observation, recording, and reporting to the physician any reaction to treatment or changes in the client’s condition.
• Participate in the team process through cooperative efforts with team members in development of the POC and team conferences and provide comprehensive, individualized care to assigned patients.
What You Will Need:
*Current NC license as Occupational Therapist and in good standing required.
*Current BLS certification required.
*Valid drivers’ license and current automobile insurance required
* Is a graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy.
* Previous Home Health Experience Required
* Must be familiar with Home Health EMR - Oasis
Job Summary:
Responsible for the evaluation, planning, directing and administering of occupational therapy modalities of treatment as prescribed by a licensed physician. Administer treatments and occupational agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. Assist patients to reach their maximum performance and level of functioning, while learning to live within the limits of the capabilities
