You Will Be Responsible For:

  • Demonstrates expert level functional knowledge of one or more of the assigned applications.
  • Prepares training documentation and provides education on new applications or functionality. 
  • Conducts train-the-trainer, super-user, and optimization  with moderate direction from the application manager. 
  • Demonstrates basic knowledge of operations within the department.            
  • Educates on Population Health applications for unique ambulatory provider/practice scenarios Serves as subject matter expert on Population Health PHSO programs.
  • Participates in team meetings, project meetings, and other meetings when requested. 
  • Collaborates with physicians, clinicians, end users, Adventist Information Technology (AIT), vendors and associates to develop and deploy electronic applications or new functionality.
  • Actively participates in support by providing solution implementation, activation, and optimization.
  • Able to articulate complex educational topics in a way which facilitates understanding, both amongst providers as well as other PHSO or AIT team staff. 
  • Uses knowledge to help connect key concepts and expectations, ultimately presenting the technologies, educational opportunities, and efficiencies in a manner that is simple for providers and staff.
  • Facilitates practice change management in regards to emerging trends related to population health initiatives, reimbursement models, patient satisfaction, customer outreach, panel growth, and provider performance with quality and efficiency metrics. The Educator should be seen as an asset to their practices by providing important information and recommended enhancements to help ensure success in the coming value-based healthcare environment with their healthcare technologies.
  • Works seamlessly, effectively, and in harmony with all internal teams including, but not limited to, Physician Enterprise (PE), PHSO credentialing, provider resolution center, quality and care integration, provider engagement optimization, analytics, and finance to ensure integrated operations that result in a superior consumer experience for the provider.
Qualifications

What You Will Need:

  • College Degree (AS, B.A., B.S. or similar) in a field of study such as business, IS, education, or a healthcare related area
  • Yearly participation in ongoing education either formal or through conferences, active professional organization participation, or journal presentations.
  • 4+ years’ experience working collegially with physicians

Job Summary:

This position will act as a Population Health subject matter expert, providing consultation and education to physicians.  The educator will create training materials, communication, and workflows to provide one on one and classroom education sessions.

Population Management Educator are expected to make regular visits to physician offices, with the intent to educate on a wide variety of topics such as population health management strategies, clinical data integration, Electronic Health Records, and Population Management tools. 

Educator will perform duties with moderate direction from their manager. Key to success is the ability to recommend workflow and systems education to ambulatory practices. Educator must be able to articulate complex topics in a way which facilitates understanding, both to providers as well as other Population Health Services Organization and Information Technology teams.

Educator will function as subject matter experts to various audiences through communication and educational initiatives to connect key concepts and expectations, ultimately presenting the technologies, educational opportunities, and efficiencies in a manner that is simple for providers to relate to and drives value to the ambulatory practice.
