Work Hours/Shift:    Full Time / Day 

You Will Be Responsible For:

  • Available to cover non-clinical functions of the Specialty Program Care Navigator when needed
  • Shared oversight accountability of the Specialty Program Care Navigator(s) with the Clinical Executive Director. Shares responsibility for setting goals, coaching, annual reviews, and required discipline
  • Collaborate with Outcomes and Quality team to collect data and develop surveys to help gauge successes and areas of improvement.
  • Liaison between the program and internal/external customers
  • Improve the patient referral system to recruit more patients into program
  • Act as host for providers during events or site visits
  • Develop presentations and speak publicly on behalf of the program
  • Institute conference management, including hotel reservations, travel, CME, marketing, vendor sponsorships, grants, educational planning with physicians, itinerary creation, prospectus development, and other conference related planning items
  • Oversees and coordinates schedules, activities and special events.
Qualifications

What You Will Need:

EDUCATION AND EXPERIENCE REQUIRED:

  • Associate Degree
  • Two (2) years’ experience in health care or patient care setting

KNOWLEDGE AND SKILLS REQUIRED:

  • Effective oral and written communication skills
  • Excellent communication and interpersonal skills.
  • Ability to speak confidently with physicians and leadership
  • Knowledge of medical terminology
  • Proficiency in Microsoft Word, PowerPoint and Excel.
  • Initiative, resourcefulness and problem-solving skills in applying detailed knowledge.
  • Ability to analyze data and create actionable improvement plans
  • Ability to coach and develop front-line team members
  • Self-sufficiency in project development management and delivery.
  • Knowledge of insurance verification, authorization process, and scheduling of procedures/surgeries

EDUCATION AND EXPERIENCE PREFERRED:

  • Bachelor’s Degree in Business, Marketing, Nursing, or Healthcare related field
  • Event planning experience
  • Formal project management experience

JOB SUMMARY:

The Program Coordinator works to assist the Institute to develop and maintain specialty programs and destination programs that create a tightly integrated ecosystem of care for specific disease states across the entire division. The Program Coordinator will be responsible for one or more specialty programs. These programs accelerate innovation, promote quality, improve safety, elevate the brand promise of AdventHealth, and aid in attracting and retaining world class physicians. They work with cross-functional teams at the Division level to assess current key programs, help to develop and manage a roadmap which includes branding, marketing, conferences, media opportunities, and identify and help maintain industry partnerships.

The Program Coordinator is responsible for collaborating with the Executive Director of Clinical Strategy to elevate the execution and performance of the Specialty Program Care Navigators by creating accountability, growth, and coaching conversations with the Specialty Program Care Navigator. Assists the Executive Director of Clinical Strategy (or other delegate) in the preparation and structured review of performance as required by the organization. They will be available to cover the patient-facing functions of the Navigator when necessary. The Navigator is the patient-facing team member of the program who is responsible for coordinating the patient through the entire continuum of care. This includes all related appointments, obtaining all necessary authorizations, remaining in constant contact with the clinical team, discharge phone calls, and post discharge follow-up. Responsible for answering patient’s questions pertaining to specialty program navigations, scheduling, and insurance information.  Serves as first responder to patient issues and works to find solutions to any barriers that arise.  The Navigator will be available to assist with patients who interact with both private and employed physician entities. Coordinates orders for procedures and follow-up imaging, obtaining and uploading into system as necessary.  The navigator oversees scanning all procedure documents and notes into Cerner, Aria, Athena, and other related EMRs.  Assists in weekly multidisciplinary meeting preparation and follow-up.  Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

The coordinator works within a matrix management environment working closely with Institute Leadership, Medical Staff, Industry Partners, and multidisciplinary teams to elevate performance of the program. The Program Coordinator tracks, analyzes, and reports out on program performance in partnership with the Outcomes & Quality team.
