Work Hours/Shift:   Full Time, Monday – Friday, 8:00am - 4:30pm


You Will Be Responsible For:

  • Conducts nutrition assessments for patients within the Care Transitions Program.  Utilizes assessment and evaluation techniques that consider the varied needs of age-specific populations as well as cultural, religious, and ethnic concerns. 
  • Develops and implements an individualized plan for nutrition intervention, revising as needed, in accordance with the patient’s medical program goals and objectives, nutrition prescription, and self-management training.   Follows up with patient at location of post-discharge transfer.  Assesses educational needs and provides nutrition counseling for individuals and groups, patients and families.  Follows up to assure that services are provided and that conditions improve.
  • Incorporates patient, physician, and customer needs and concerns into decision-making and organizational action.  Identifies gaps in service that prevent the patient from achieving increased stability in daily living.  Acquires working knowledge of motivational interviewing and working with resistant clients.
  • Evaluates the effectiveness and measures progress towards outcomes of medical nutrition therapy interventions.  Reassesses nutrition care process and implements changes as indicated.  Evaluates practice upon completion of case intervention, determining whether intervention was successful and whether client achieved expected outcome.  Seeks appropriate consultation for patients who are at high risk for readmission or for high emergency room utilization.  Admits mistakes openly and seeks ways to resolve issues.  Creates a safe environment for honest and open communication.
  • Documents in patient’s medical record after each significant contact and at closure of case, according to AdventHealth and regulatory standards.  Provides appropriate documentation that summarizes the nutrition care, including nutrition assessment, diagnosis plan, implementation, and progress toward goals.  Provides and records ongoing data on caseload for purposes of program evaluation.


What You Will Need:

  • Bachelor’s degree in Food and Nutrition, Dietetics, Nutritional Sciences, Human Nutrition, or related field
  • One to two years of experience as Registered Dietitian                        
  • Clear/Active License as a Dietitian/Nutritionist in Florida
  • Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR)
  • Ability to apply American Dietetic Association’s Nutrition Care Process
  • Excellent decision making and judgment capabilities
  • Excellent time management and organizational skills, self-motivation, and ability to work independently
  • Proficiency with Cerner applications
  • American Heart Association (AHA) Basic Life Support (BLS) certification
  • Master of Science in Food and Nutrition, Dietetics, Nutritional Sciences, Human Nutrition, or nutrition related field
  • Experience in an acute care outpatient setting
  • Three to five years of Experience as a Registered Dietitian
  • Certified Nutrition Support Dietitian (CNSD) OR Certified Diabetes Educator (CDE) OR Renal Certification
Job Summary:

The Care Transitions Dietitian is responsible for ensuring that a care plan is carried out in partnership with the patient.  Works as part of the interdisciplinary Care Transitions Team to implement the AdventHealth’s readmission prevention programs, which are targeted to reduce the number of patients who are readmitted to the hospital.  Follows selected patients who transition from the hospital to a lower level of care and participates in routine readmission meetings with community partners as well as comply with data collection expectations.  Works as part of various other teams, including but not limited to: Care Management nurses and social workers, nursing, home care liaisons, physicians, pharmacists, dietitians, and leadership.  Adheres to the Care Management Scope of Service in achieving the goals of the Care Transitions Team.  Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
