You Will Be Responsible For:

ASSESSMENT:

  • Proactively intervenes in complex patient situations pertaining to the Cardiology patient, which results in improved patient outcomes with consideration of cultural sensitivity and unique learning styles.
  • Assess organizational status in relation to assigned core measure(s)
  • Performs concurrent data review and rounding for assigned patients within designated timeframe
  • Communicates information gathered from concurrent medical record review to the appropriate leaders for analysis and corrective action to improve quality outcomes
  • Assures concurrent review is performed on those patients that may trigger assigned evidence-based measure care paths

NAVIGATION:

  • Serves as a singly point of contact for referring physicians, patients, and caregivers within the program
  • Identifying patients overdue for visits, labs, or referrals and arranging for follow-up services as appropriate
  • Identifying patients not meeting clinical goals, such as EKG, BMI, BP or glucose control, and arranging for follow- up visits accordingly
  • Conducts patient chart review including identification of all needed preventive health maintenance, i.e.
  • Endoscopy, mammography, pulmonology, diabetes, etc.
  • Helping patient identify and overcome barriers
  • Provides or arranges needed patient education regarding specific health needs
  • Acts as a liaison with patients and the clinic/ hospital.
  • Intermittent phone calls for preparatory and follow up information.

PROGRAM DEVELOPMENT:

  • Develops statistical sampling framework that assures appropriate sampling in relation to significant review of
  • Medical records pertaining to assigned cardiology health patients.
  • Program development includes the following steps as they relate to driving outcomes as they relate to core
  • measure(s) assigned:
  • Assessment
  • Implementation
  • Evaluation
  • Modification
  • Develops evidence-based programs or workshops based on needs assessments and interpretation of quality data that include (but are not limited to) topics below:
  • Emergency issues and interventions related to the cardiology patient
  • Develops programs for unit or group specific, based on identified educational needs for staffing/nursing
  • Works with medical staff to provide real-time education for colleagues, to improve quality of care, as it relates to cardiology clinical pathways
  • Documents the planning and implementation of program for compliance
  • Engages and interacts with medical staff, resident physicians, allied health professionals, and nursing staff to meet the necessary objectives of the standards of care for cardiology care.

CLINICAL DEVELOPMENT:

  • Serves as an educational resource for internal and external audiences by developing educational material and delivering presentations.
  • Collects preoperative, intraoperative, 30-day postoperative, and long-term (six month and annual thereafter) follow-up data components for the program through the effective utilization of the hospital medical record systems and through a coordinated effort with individual surgeon office staff.  
  • Identifies, streamlining and process improvement opportunities in the data collection process.
  • Researches all re-admits, complications and prolonged LOS and reports out at monthly meetings.
  • Provides analytical assistance for research projects.  Participates in data compilation for audits and initial and reaccreditation site visits.  Utilizes software applications for data collection and analysis.
  • Attends Cardiology surgical staff meetings, interdisciplinary clinical meetings, quality improvement meetings, patient safety and quality meetings.  Assists with specialized cardiology functions, informational sessions, and program teleconference.

 

RESEARCH/EVIDENCE BASED PRACTICE:

  • Recognizes goal of education initiatives that empower nursing staff through education initiatives that
  • ultimately improve patient outcomes
  • Communicates all corporate mandates related to assigned core measure(s) to leadership team
  • Participates in research activities appropriate to level of education and experience – as they relate to care of
  • the cardiology patient
  • Review and assess all new requirements and alert appropriate personnel to new guidelines/criteria that
  • pertain to care of the cardiology patient

  • based measures/meaningful use, as they relate to the cardiology patient

  • Graduate of an accredited School of Nursing with Associates Degree in Nursing. 
  • Minimum of three years of clinical experience in a specialized area, such as cardiology, cardiovascular surgery, hearth health management or wellness, within a healthcare setting.
  • Current licensure as a Registered Nurse in Florida or Licensure from another state with verification of application and eligibility for Florida licensure by endorsement.
  • BLS certifications

Job Summary:

  • Specialty RN responsible for the supervision and implementation of the Cardiology Program; Conventional and Interventional Cardiology. The Navigator will work closely with the surgeons and their office staff to ensure an uneventful and successful progression for the patient through the entire surgery process.  Program to be based on knowledge inherent in the Cardiology Health scope.  Specifically coordinating the patient pathways through Cardiology Health management, with utilization of a multidisciplinary approach. Program development is organizational wide for specific advancement towards acquisition of or maintenance of any applicable accreditation protocols.  Plans, teaches and through summative and formative evaluation, reviews clinical pathways for nurses, leaders, physicians, and clinical staff.  Participates in and uses evidence-based practice and research to promote professional development and optimal patient outcomes related to managing the cardiology patient.  Acts as the cardiology health expert for the organization in relation to assigned core measure(s) and pertinent regulations.  Ability to statistically coordinate sampling of medical records that pertain to the cardiology health patient throughout the organization.  Ability to evaluate, summarize, and disseminate data to appropriate personnel in the organization.  Works collaboratively with all disciplines in the organization to establish and implement action plans for resolution of identified areas of concern.
