The role you’ll contribute:

The Targeted Case Manager is responsible for conducting a comprehensive assessment and periodic reassessment of an individual’s strengths and needs and assist them in gaining access to identified medical, social, educational, and other needed services and support.

The value you’ll bring to the team:

  • Assist patients in obtaining needed referrals and support based on identified needs

  • Develop a strength and need based assessment and periodic reassessment for each patient served

  • In accordance with state and federal criteria

  • Develop an individualized care plan for each patient served in accordance with state and federal criteria.

  • Work with the patient and/or patient’s family to address issues related to implementation of their care plan.

  • Assess the effectiveness of the care plan in meeting the identified needs of the recipient and make changes as needed

  • Link the recipient with services and resources identified in the care plan

  • Advocate for the acquisition of services and resources necessary to implement the care plan

Qualifications

The expertise and experiences you’ll need to succeed:

Minimum qualifications:

  • High School Education or equivalent

  • Bachelor of arts or science degree in a behavioral science from an accredited university

  • One year full-time employment working directly with individuals in a human service setting

  • Completion of 6 hours continuing education annually reported to the board every 3 years

  • Completion or agree to complete AHCA-approved targeted case management training within three

  • Months of initially providing services

  • Completion of specialized training for the targeted population being served by the case manager

Preferred qualifications:

  • Master’s degree in behavioral science
