The role you’ll contribute:
The RN Patient Care Coordinator is responsible for coordinating the care of patients within their specialty. Responsible for ensuring the full continuum of patient care is being met.

New patient consults and patient work up coordination
Appointments and scheduling with continuation of care
Insurance authorizations and out-of-network insurance benefits
Surgery coordination and liaison with patient, physicians, and referring/other physicians in the community, APPs, and anesthesiologists as required.
Initiates, facilitates and monitors the development of patient care pathways that identifies critical factors for achieving desired outcomes for discharge, post-hospitalization recovery, and health maintenance and promotion for a specific patient group.
Performs nurse visits, and augments care given by the providers in the office or in the OR.
Acts as a resource person for primary physicians, referring physicians, staff, nurses, and other members of the healthcare team.
May Provides nursing triage services and health advice for patients over the phone with appropriate documentation.

Bachelor’s Degree in Nursing (BSN)
Care Coordination Experience
Relevant specialty experience
