The community you’ll be caring for: AdventHealth Hospice Care

The role you’ll contribute:

The Bereavement Coordinator provides various bereavement follow-up services to HospiceCare families/caregivers for at least thirteen months after the patient's death. Services are directed toward individuals experiencing normal grief reactions with the goal of providing support and education to promote healthy grieving and minimize negative physical, emotional, social and spiritual consequences of this time. The Bereavement Coordinator works closely with the interdisciplinary team and at times provides Hospice Social Work services. The Bereavement Coordinator is responsible for assessing risk levels of patients and families for follow up services. The Bereavement Coordinator is responsible for required documentation per Hospice Medicare COPs and Memorial services

The value you’ll bring to the team:

  • Provides bereavement support and grief education for individuals who have lost a family member and leads support groups, promotes intra-group discussions, and encourages identification and expression of feelings in a safe environment.

  • Contacts and assesses Primary caregivers of Advent Health Hospice of East Florida patients within 18 days of the patient’s death.

  • Offers brief therapy specifically related to grief issues of families to assist clients in developing support systems and coping skills. Informs clients of available community resources to empower clients to seek support in the community once the six bereavement counseling sessions have concluded or if the client’s needs exceed what can be provided in short term bereavement counseling.

  • Provides individual and group support for both hospice and community clients in the office, in the form of tele visits and in the field. Will travel as needed to participate in designated support groups and social activities.


The expertise and experiences you’ll need to succeed:

  • Master’s degree in Clinical Social Work, Marriage and Family Therapy, Mental Health Counseling, or related field is required. An equivalent combination of education and experience may be substituted at the discretion of the Administrator.

  • Experience in hospice or related health care field is required.

  • LCSW or actively working toward a LCSW is preferred.

  • Active Florida driver’s license and insurance.
