The role you’ll contribute:

The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.

  • Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to thepatient from first contact and throughout the continuum of care.

  • Guide patients on treatment plans, next steps in appointments (i.e. surgical prep, imaging, labs, tests, which provider to see next, etc.)

  • Assist with scheduling follow-up appointments and provide information and reminders to patients& physicians’ offices.

  • Ensures an extraordinary customer experience.

Qualifications

The expertise and experiences you’ll need to succeed:

  • Bachelor's degree in nursing

  • Three years of experience required; one year working with breast cancer patients preferred

  • Strong psychosocial assessment and clinical skills

  • CO or Compact RN license

  • Basic Life Support Certificate through the American Heart Association
