The role you’ll contribute:

Social work practice consists of professionally responsible intervention to 1) enhance the development, problem-solving, and coping capacities of people, 2) promote the effective and humane operation of systems that provide resources and services to people, 3) link people with systems that provide them with resources, services, and opportunities, and 4) contribute to the development and improvement of social policy, generally and specifically health and welfare policy in the primary care setting and community. The focus of the social work assignment and intervention is on patients referred by a primary care provider or clinical staff due to at risk status or for unmet psychosocial needs

The value you’ll bring to the team:

• Function as a treatment team member, interpreting social, psychological, emotional and family system problems and strengths to other members of the treatment team

• Prioritize clinical problems, formulate treatment goals and construct treatment plan, revising as needed, based on continuous evaluation and assessment of progress

• Incorporates patient, physician, customer needs and concerns into decision-making

• Demonstrate ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge

Qualifications

The expertise and experiences you’ll need to succeed:

• Master’s degree

• Up to 1 Year of Position-Related Experience

• Kansas Licensed Master Social Worker (LMSW) OR

• Kansas Licensed Specialist Clinical Social Worker (LSCSW)
