Job Description:

  • Oversees the team's utilization of technology and adherence to standard work to develop and implement effective discharge plans in collaboration with patients, families, care teams, and payors.
  • Participates in the hiring, onboarding, education, and retention of staff. Participates in hospital meetings and initiatives to improve the consumer experience and achieve organizational goals.
  • Coordinates and leads family and care team meetings as needed.
  • Monitors for process improvement opportunities and participates in performance improvement initiatives and reports quality/risk issues to appropriate parties.
  • Collaborates and problem solves with outside agencies to facilitate patient transitions to post-acute settings.

Knowledge, Skills, and Abilities:

• Understanding of core Care Management principles and CMS Conditions of Participation [Required]
• Knowledge of medical necessity for appropriate patient status and level of care [Required]
• Understanding and ability to implement mitigation techniques to attain hospital metrics [Required]
• Ability to analyze, evaluate, and monitor data [Required]
• Ability to foster positive Interdisciplinary Team relations and encourage employee engagement [Required]
• Strong professional verbal and electronic communication [Required]
• Ability to work independently and/or part of a team [Required]
• Strong technology skills Required; including working knowledge of Microsoft PowerPoint Preferred and intermediate knowledge of Microsoft Excel [Preferred]
• Understanding of Managed Care and Value Based Programs [Required]
• Ability to negotiate optimal resolution of issues [Required]
• Ability to communicate effectively with diverse populations [Required]

• Bachelor’s [Required]
• Master’s [Preferred]

Field of Study:

• in Nursing or Social Work

Work Experience:

• 1+ years cm leadership experience [Preferred]
• 1+ years leadership experience [Required]
• 1+ years nursing experience [Required]
• 4+ years care management experience [Preferred]

Additional Information:

• If RN: Bachelor’s of Science in Nursing [Required] OR

• If LCSW: Master’s in Social Work [Required] OR

•  Bachelor’s in Social Work and Master’s degree [Required]

Licenses and Certifications:

• Registered Nurse (RN) [Required] OR

• Clinical Social Worker License (LCSW) [Required]
• RN-BC Case Manager [Preferred]
