Job Description:

Acts as a liaison with physicians’ offices to ensure proper documentation and orders are in place. Develops connections within the organization to ensure timely and effective patient care. Other duties as assigned. Develops, plans, and implements evidence-based measures to enhance the quality of care for high-risk hereditary breast, ovarian, and colon cancer patients. Coordinates multidisciplinary services to ensure comprehensive and cost-effective care. Serves as a resource for patients, physicians, and staff, providing education and consultation. Establishes professional relationships with patients and families to guide them through diagnosis, treatment, and follow-up. Assesses patient status and collaborates with physicians to develop appropriate care plans. Collects and analyzes data for quality improvement and performance trends. Works with the Genomic and Personalized Health Team to integrate personalized care based on genetic risk. Communicates patient status and needs to the multidisciplinary team. Provides ongoing patient education and support throughout the care continuum. Facilitates scheduling and coordination of necessary tests and procedures.Knowledge, Skills, and Abilities:
• Effective verbal and written communication skills [Required]• Ability to read, write and explain moderately complex information to hospital and clinical staff with a great deal of attention to detail [Required]• Ability to communicate effectively in a supportive manner to patients, family, staff and physicians [Required]• Ability to explain information to patients and families in a way the patient understands, and/or explain departmental or hospital procedures to employees, visitors and patients [Required]• Ability to observe assess, guide and evaluate others [Required]• Ability to communicate timely and in an open and honest manner [Required]• Ability to collect data, interpret findings, set priorities and carry out established plansFlexibility in prioritization; ability to multitask; critical thinking/problem-solving skills [Required]• Self-motivated with ability to work independently and in a group setting [Required]• Strong organizational skills [Required]• Proficient in Microsoft Office, (Outlook, Word, Excel, Power Point), and Teams [Preferred]• Knowledge of EMR e.g., Cerner/EPIC, Athena/EPIC and associated systems [Preferred]

Education:
• Bachelor’s of Nursing [Required]• Master’s [Preferred]

Field of Study:
• in Nursing, Business Management or Healthcare Management

Work Experience:
• 3+ years of experience in oncology or navigator role experience [Required]• Previous experience with projects or program start-up [Preferred]
