Job Summary

The Clinical Pharmacist supports the mission of the department, hospital, and health system by preventing, identifying, and resolving medication-related issues to promote patient safety, improve clinical outcomes, enhance patient satisfaction, and optimize resource utilization. This position provides comprehensive pharmacy services in both centralized and decentralized practice settings and performs all functions of a licensed pharmacist, including medication order review, dispensing, clinical consultation, patient education, and supervision of pharmacy personnel. The Clinical Pharmacist collaborates with interdisciplinary healthcare teams to ensure safe, effective, evidence-based medication therapy and supports quality improvement, regulatory compliance, and operational excellence initiatives.

Essential Functions

• Reviews, verifies, interprets, and processes medication orders to ensure safe, accurate, and appropriate medication therapy.

• Evaluates patient-specific factors including diagnosis, laboratory values, allergies, age, renal and hepatic function, medication history, and clinical status to optimize medication management.

• Identifies, prevents, and resolves medication-related problems, adverse drug events, drug interactions, therapeutic duplications, and medication errors.

• Performs pharmacokinetic dosing, therapeutic drug monitoring, medication adjustments, and pharmacist consults in accordance with established protocols.

• Compounds, prepares, dispenses, verifies, and documents medications in accordance with organizational policies, regulatory requirements, and professional standards.

• Participates in interdisciplinary rounds and collaborates with physicians, nurses, case managers, and other healthcare professionals to develop and optimize medication treatment plans.

• Provides medication education and counseling to patients, families, caregivers, and healthcare team members regarding safe and effective medication use.

• Supports transitions of care initiatives from admission through discharge and follow-up to ensure continuity of medication management.

• Participates in antimicrobial stewardship, medication surveillance, formulary management, and restricted medication programs.

• Serves as a drug information resource for patients, providers, and healthcare team members.

• Supervises, mentors, trains, and evaluates pharmacy technicians, pharmacy interns, students, residents, and other pharmacy personnel.

• Utilizes electronic health records, pharmacy information systems, clinical decision support tools, and medication automation technologies to promote safe and efficient medication use.

• Identifies, documents, reports, and participates in the resolution of medication safety events, adverse drug reactions, and medication variances.

• Participates in quality improvement initiatives, performance improvement activities, regulatory compliance efforts, and accreditation readiness activities.

• Monitors and tracks clinical interventions, medication safety outcomes, quality metrics, cost-saving initiatives, and departmental performance indicators.

• Contributes to professional advancement through committee participation, research activities, specialty practice development, continuing education, and scholarly activities.

• Assists with daily pharmacy operations to ensure effective workflow and efficient patient care services.

• Performs other duties as assigned.

• Proficiency with computer applications, keyboarding, and pharmacy-related technology systems.

• Commitment to continuous learning, professional development, and advancement of pharmacy practice.

Education

• Bachelor of Science in Pharmacy required.

• Doctor of Pharmacy (PharmD) preferred.

Field of Study

• Pharmacy.

Work Experience

• Hospital pharmacy experience preferred.

• One year of hospital pharmacy experience strongly preferred.

Licenses and Certifications

• Licensed Pharmacist (RPh) required.
