Job Description:
Schedule: Sun- Thursday 0830 – 1700
- Processes all referrals to the Rehabilitation unit for appropriateness, identifies discharge planning needs, and ensures patients meet criteria for inpatient rehabilitation per CMS guidelines.
- Responds promptly to all referrals, communicates the decision to admit or deny to the referral source, and completes the pre-admission screen with required clinical information, verifying medical and rehabilitation needs, insurance coverage, and discharge plans.
- Upon collaboration with the Rehab physician as appropriate and receiving concurrence or denial, the decision is communicated to the patient, family, and healthcare team. Evaluates patients' needs for additional assistance with medical equipment, oxygen therapy, IV therapy, emergency equipment, pharmaceuticals need, specialty items, etc., during the discharge planning process and assures availability of equipment by providing ample time to the inpatient rehabilitation unit to order specialty items.
- Works collaboratively with UR, Case Management, Social Services, financial counselors, and insurance carriers to expedite patients' discharge to the rehabilitation unit effectively and efficiently.
- Notifies the patient, family, or healthcare surrogate of their financial obligations and documents patient interviews, evaluations, recommendations, and summaries in the medical record.
- Demonstrates competency in all procedures within the scope of practice, appropriate to the ages of the patients served, and maintains knowledge of growth and development with an understanding of the range of treatment needed by these patients.
- Willingly adjusts and adapts to changes in workload by altering working hours to assist in other areas as approved and observed by the manager/director.
- May conduct on-site visits with referring healthcare individuals and/or organizations for the purposes of assessing candidates and introducing or reviewing the various programs available at the Rehabilitation Hospital. Communicates and/or consults with the chain of command regarding admissions that do not meet criteria or need further medical/technical review.
- Responds quickly to issues identified by referral sources, skillfully communicating to the appropriate entity, and demonstrates skill and persistence in overcoming objections while identifying and following through with opportunities.
- Effective in identifying referral opportunities and develops an action plan with leadership to capitalize on these opportunities.
- Looks for opportunities to reduce referral source workload relative to all aspects of the referral/admission process.
- Responsible for developing meaningful personal relationships with care management and physicians using customer profiles and is successful in securing face-to-face meetings with key decision-makers in the referral process.
- Articulates issues facing referral sources and potential patients, which can impact admissions, and suggests potential solutions.
- Contributes to the monitoring of the quality and appropriateness of patient care delivered and reports problems or concerns regarding the quality and appropriateness of patient care to the Manager/Director.
- Works with marketing to align organization marketing goals and activities.
- Establishes professional relationships with healthcare individuals and organizations that refer candidates for inpatient rehabilitation, supports various clinical aspects of inpatient rehab care, and establishes and maintains methods of communication with referring and supporting healthcare individuals and organizations to ensure a clear and concise exchange of necessary information in a timely manner.
- Performs other duties as assigned.
Knowledge, Skills, and Abilities:
- Understands and can communicate effectively with both external and internal stakeholders regarding the continuum of care [Required]
- Must be able to multi-task and effectively manage multiple referral sources [Required]
- Possesses excellent organizational and communication skills, demonstrating a commitment to patient and partners’ oriented practice [Required]
- Good computer skills, including proficiency in Word, Excel, Outlook, and basic knowledge of Microsoft Office and Windows [Required]
- Ability to read, write, and speak conversational English effectively [Required]
Education:
- Associates [Required]
- Bachelors [Preferred]
Field of Study:

Work Experience:
- 2+ year experience in clinical setting [Required]
Licenses and Certifications:
