Job Description:
* Conduct comprehensive patient assessments to gather data on physical, emotional, and social health status. * Develop and implement individualized care plans based on assessment findings, setting goals and outlining interventions. * Administer medications and treatments accurately as prescribed, ensuring patient safety and adherence to protocols. * Monitor and evaluate patient responses to interventions, adjusting care plans as necessary and documenting progress in the electronic medical record. * Collaborate with multidisciplinary teams to resolve patient care issues and ensure coordinated, comprehensive care. * Educate patients and families about health conditions, treatment plans, and self-care strategies to empower them in managing their health. * Supervise and guide nursing personnel in delivering safe, therapeutic, and quality patient care, maintaining a professional rapport with all team members. * Ensure safe and effective handoff communications by using standardized communication tools during shift changes and patient transfers. * Perform and oversee complex patient care interventions while maintaining aseptic technique and infection control practices. * Provide discharge planning to ensure a smooth transition from hospital to home or another care setting, addressing the patient’s needs and resources. * Engage in ongoing education and professional development to maintain clinical competencies and improve patient care. * Maintain detailed records of patient care activities to support continuity of care and prevent errors.Knowledge, Skills, and Abilities:
