Job Description:
- Reviews reports on Current Core Measure Hospitalizations and Re-admissions daily.
- Coordinates the Readmission Prevention Committee Meetings, identifies recent readmission cases, reviews for improvement opportunities, creates and sends out meeting agenda, and completes meeting minutes.
- Participates in in-patient visits and post-discharge phone calls, utilizing data reports to identify patients with Core Measure diagnoses.
- Visits patients in the hospital, provides education on disease process and medications, and collaborates with Care Management to address discharge needs.
- Collaborates with Respiratory Therapy and Cardiopulmonary Rehab Teams when appropriate.
Knowledge, Skills, and Abilities:
• Demonstrates proficiency in nursing care, knowledge, and skills [Required]• Demonstrates ability to lead, support, and build on current efforts of the readmission reduction program [Required]• Demonstrates initiative, flexibility, integrity, and diplomacy [Required]• Demonstrates strong professional, organizational, and interpersonal skills for effective and creative leadership in working with all levels of the organization including physicians, committees, senior management, and trustees, as well as patients and their families [Required]• Demonstrates effective communication skills [Required]• Proficient use of Excel, Microsoft Word, Microsoft PowerPoint, and Microsoft Teams [Required]• Experience in working with EMR [Required]• Working knowledge of current care practices for the diagnostic groups identified by CMS [Preferred]• Working knowledge of Federal and State regulatory requirements in hospital accreditation [Preferred]
Education:
• Associate's of Nursing [Required]• Bachelor's of Nursing [Preferred]

• 1+ years acute care clinical experience [Required]• 2+ years acute care clinical experience [Preferred]
