Job Description:

• Encourages patient engagement in healthcare partnership through motivational interviewing, evidence-based self-management support protocols, tools, and teach-back methodology.
• Manages resources and patient flow by identifying community resources to support wellness with individualized patient needs.
• Participates in continuous process improvement by taking an active role in patient experience and satisfaction.
• Other duties as assigned.
• Documents ongoing status, interventions, patient response, and outcomes in an accurate and timely manner.
