Job Description:
Other duties as assigned. Coordinates and participates in multidisciplinary conferences, tracks scheduled procedures, follows pathology results, and monitors outcomes of meetings. Serves as a resource to cancer patients, educates and answers questions about disease-specific health concerns, and participates in health lectures and educational outreach programs. Educates patients regarding expectations before, during, and after diagnostic and treatment procedures, and develops and revises patient educational materials. Builds relationships with community physician practices/groups to grow disease-specific inpatient and outpatient market share. Promotes the Lung Cancer Care Navigation Pathway and multidisciplinary program to community and outreach programs. Researches and develops tools and strategies to promote the Multidisciplinary Lung Program Pathway to area physicians. Visits physician offices regularly, handles physician/physician office issues promptly, and initiates physician communications as needed. Ensures physician satisfaction with services and presents concerns to administrative leadership on an ongoing basis. Facilitates the navigation of cancer patients through the pathway program, ensuring efficient and coordinated services through screening, diagnosis, treatment, and survivorship phases. Establishes and maintains positive communication and relationships with referring physicians, pulmonologists, surgeons, radiologists, pathologists, radiation oncologists, and medical oncologists. Facilitates patient continuity of care between hospital departments and provides patient follow-up for laboratory, radiology, and pathology procedures. Engages and enrolls employed primary care physicians in the multidisciplinary lung cancer care program.Knowledge, Skills, and Abilities:
• Strong interpersonal skills, excellent conflict resolution and communication skills and computer proficiency [Required]
• Must have established positive working relations with the medical community [Required]
• Leadership, mentorship/clinical resource skills [Required]
• Ability to multitask on a variety of tasks and projects [Required]
• Must have knowledge of the DNV requirements and local, state and federal regulations [Required]
• Must have knowledge on NCCN, AJCC and other standard oncology clinical management [Required]
Education:
• Associate’s of Nursing [Required]
• Bachelor’s of Nursing [Preferred]
Field of Study:
• N/A
Work Experience:
• 1+ critical care experience [Required]
• 2+ diagnostic imaging rn experience [Preferred]
• Healthcare marketing and business development work experience [Preferred]
• Must have experience in hemodynamic monitoring [Required]
• Oncology and/or radiation clinical experience [Preferred]
