Job Description:

• Makes and answers calls in a professional manner with kindness and empathy. Follows scripted questions provided to ensure consistency and accuracy. Responds to customer inquiries and notifies Guest Relations to resolve customer complaints.
• Identifies and escalates priority issues discovered during calls.
• Routes any immediate needs to the appropriate resource for timely resolution. Informs and educates patients regarding the HCAHPS survey to improve patient satisfaction.
• Attempts to contact 100% of patients discharged, making up to three attempts per patient. Completes call log data accurately and produces call log reports for review.
• Provides case management in urgent situations to ensure patient needs are met. Ensures timely communication with directors, guest relations, physicians, or home health services. Supports patient care continuity and satisfaction through effective follow-up. Performs other duties as assigned.

Education:

• Associate's of Nursing [Required]

• Bachelor's of Nursing [Preferred]

Work Experience:

• ER, ICU, Critical Care experience

• Discharge Call Back experience

Additional :

Licenses and Certifications:

• Registered Nurse (RN) [Required]

• Basic Life Support – CPR Cert (BLS) [Required]

$28.18 – $45.09
