Job Description:

Schedule: Full Time, 36 hours per week.

Shift: Night Shift, 6:45pm-7am/ 7pm-7:30am. Orientation and training will be on days.

Location: All of Orange, Osceola, and Seminole Counties. Main campus office is in Altamonte Springs

Provides skilled nursing care to hospice patients in private homes, skilled nursing facilities, assisted living facilities, and inpatient units. Assesses patient conditions and symptoms to determine the need for crisis care. Administers medication, takes vitals, feeds, bathes, changes clothes and bedding, dresses wounds, and provides other palliative care as indicated in the care plan. Offers support and education to the patient's family and caregivers. Communicates with supervisory RN regarding symptom management, changes in patient condition, safety issues, and other patient care concerns. Completes all required documentation for the patient's medical record, including medication profiles. Ensures charting documents patient condition, medications administered, changes in condition, directives from supervisors, safety issues, and caregiver education. Performs post-mortem procedures according to policy and completes required documentation. Provides brief respite for caregivers during shifts as needed. Participates in staff, department, team meetings, in-services, and other related activities. Attends educational and developmental seminars and meetings as appropriate. Interacts appropriately with caregivers and staff in the patient's place of residence. Responds promptly to pages and phone calls during assigned hours of duty. Other duties as assigned.

Knowledge, Skills, and Abilities:
• Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]• Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]• Knowledge of electronic medical record. [Required]• Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form. [Required]• Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]• Ability to complete AdventHealth Hospice Care Orientation and Training, as necessary. [Required]
