Job Description:
Performs clinical and socio-economic assessments on referred patients and navigates care between physicians and diagnostic services. Connects patients and families with resources for education, support, and clinical needs throughout their course of treatment. Works collaboratively with Patient Accounting and Coding to ensure compliance with patient charges and appropriate coding of procedures. Provides effective follow-up with structural heart patients per mandatory registry guidelines and ensures records are in EMR for registry data abstraction. Coordinates and hosts weekly multi-specialty physician team meetings, formulates agendas, and follow-up recommendations to facilitate patient care. Uploads required data to appropriate vendors and maintains communication with device representatives. Facilitates scheduling and authorization of procedures, both pre and post-procedure. Actively engages with Marketing to enhance referrals and provide community education. Other duties as assigned. Receives physician referrals into the program, compiles clinical data, and assesses appropriateness for presentation to the multi-specialty physician team.
Exempt RN Position: Monday -Friday 8am-430pm.

The Structural Heart Coordinator (RN) will oversee all aspects of the Transcatheter Aortic Valve Replacement (TAVR) program. Responsibilities include coordinating patient evaluation, facilitating multidisciplinary team communication, ensuring compliance with regulatory and program requirements, and supporting continuity of care across the procedural pathway. The RN will serve as the primary point of contact for patients, providers, and ancillary services throughout pre-procedural, intra-procedural, and post-procedural phases. Additionally, the coordinator will monitor clinical outcomes, maintain program data, and contribute to quality improvement initiatives related to structural heart services.
Knowledge, Skills, and Abilities:
• Skilled in critical care nursing and operating room or catheterization lab experience; [Required]
• Basic cardiovascular rhythm recognition and cardiopulmonary monitoring assessment; [Required]
• Advanced knowledge of cardiovascular diagnostics and imaging techniques; [Required]
• Excellent interpersonal and communication skills, written and verbal; [Required]
• Ability to problem solve and multitask [Required]
• Highly organized in compiling clinical data / images for presentation to multi-specialty physician teams to accurately assess patients for advanced clinical procedures; [Required]
• Diplomatic personality with strong drive to grow the program [Required]
• Proficiency in Microsoft PowerPoint, Excel and Word [Preferred]
Education:
• Associate's of Nursing [Required]
Field of Study:

Work Experience:
• 1+ cardiac surgical critical care experience [Preferred]
• Registered Nurse with a minimum 5 years of experience cardiac critical care [Required]
