Job Description:

Schedule: Full Time

Shift: Requires full availability for Mon-Fri, 8am-5pm

Performs or assigns new hire training, conducts post-training assessments, and assists with ongoing clinical in-service education initiatives and remediation. Participates in department performance improvement activities, attends clinical meetings, follows up on assignments/issues, completes tasks, and reports ongoing performance indicators. Creates monthly communication tools to update the clinical team on best practices and develops pocket guides, SOPs, and flowcharts for clinical teams to follow in accordance with evidence-based practice. Leads continuous clinical training across locations ensuring standard work and policies & procedures are consistent across all sites of care. Follows CDC guidelines for sterile technique and isolation, demonstrates appropriate use of PPE, performs hand hygiene according to policy, and enforces safety regulations. Knowledge, Skills, and Abilities:

• Working knowledge of general clinical practices to include principals of primary care, nursing, and laboratory. [Required]

• Knowledgeable of chronic disease management and continuity of care to include care gap closure. [Required]

• Strong critical thinking skills [Required]

• Demonstrates professional clinical competency [Required]

• EMR Superuser (will provide training) [Required]

• Good organizational and time management skills [Required]

• Demonstrated ability to communicate by reading, writing legibly, speaking and comprehending English effectively in order to carry out job requirements [Required]

• Ability to operate a computer, copier, fax and scanner [Required]

• Ability to establish and maintain effective working relationships with patients, employees, and others of diverse backgrounds [Required]

• Ability to effectively problem solve, plan, organize, direct, advocate, teach and work independently with solid judgement [Required]

• Excellent time management and organizational skills, self-motivation, and ability to work independently [Required]

• Ability to multi-task efficiently and work in a fast pace, high demand environment [Required]

• Microsoft Office Suite [Required]

• Proficient EMR documentation [Preferred]

Education:

• High School Grad or Equiv [Required]

• Technical/Vocational School [Required]

Field of Study:

• Completion of licensed practical nurse program

Work Experience:

• 2+ years primary care experience [Required]

• 2+ years supervisory experience [Preferred]

• 4+ years primary care experience [Preferred]

Licenses and Certifications:

• Basic Life Support – CPR Cert (BLS) [Required]

• Licensed Practical Nurse (LPN) [Required]
