Job Description:

• Assists with palliative care consultations and interventions for early initiation of chronic disease management related to life-limiting conditions.
• Advocates for patients and families by initiating referrals to home care or hospice services for eligible patients.
• Provides education related to palliative care to patients, families, community, and healthcare professional groups.
• Performs initial palliative evaluation with patient and family, including health history and psychosocial determinants.
• Participates in quality monitoring, regulatory compliance, and risk management performance improvement activities to ensure continuous improvement.

Education:
• Associate’s of Nursing [Required]• Bachelor’s of Nursing [Preferred]• Master’s of Nursing [Preferred]

Field of Study:
• completion of a medical coding certification program

Work Experience:
• 3+ clinical, hospice or hospital setting. [Required]• Clinical nursing care experience in palliative care, oncology, hospice, geriatrics, or intensive care [Preferred]

Associate’s of Nursing (Required), Bachelor’s of Nursing, Master’s of NursingCertified Hospice and Palliative Nurse (CHPN) – EV Accredited Issuing Body, Registered Nurse (RN) – EV Accredited Issuing Body

Day-Weekend (United States of America)

Pay Range:

$32.76 – $57.47
