Job Description:

• All the benefits and perks you need for you and your family:  

  • Up to $2,000 Sign on Bonus 

  Schedule: Full Time 

  Shift:  Monday – Friday 8am to 5pm

  Location: Daytona Beach, FL

• Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals. 
• Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space.
• Communicates through various digital channels to support patient care. Performs initial health screens and intake for new members.
• Obtains and reviews old medical records to ensure comprehensive patient history. Identifies care gaps and orders necessary screening tests.
• Confirms appointments and provides instructions to patients.

Knowledge, Skills, and Abilities:

• Knowledge of chronic disease management [Required]

• Empathetic nature and a distinct ability to consistently provide top tier service. [Required]

• Driven by passionate desire to enhance and improve member outcomes. [Required]

• Knowledgeable in member advocacy and navigating complex systems. [Required]

• Responsible for managing the member’s entire care plan with the guidance of the RN Case Manager and Provider. [Required]

Education:

• Technical/Vocational School [Required]

Field of Study:

• Graduate of a Medical Assistant Program

• Basic Life Support – CPR Cert (BLS) [Required]

• Certified Medical Assistant (CMA) [Required] OR

• Clinical Medical Assistant Certification (CMAC) [Required] OR

• Certified Clinical Medical Assistant (CCMA) [Required] OR

• National Certified Medical Assistant (NCMA) [Required] OR

• Nationally Registered Certified Medical Assistant (NRCMA) [Required]
