Job Description:

Identifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors. Actively participates in direct patient care programs, and collaborates with healthcare providers and pharmacy staff to deliver patient-centered care. Performs all pharmacist functions, actively participating in established standard work parameters and providing coverage in central and satellite pharmacy areas. Leads pharmacy services in assigned areas. Accurately manages medication inventories, and prepares and dispenses patient-specific medication doses. Supervises pharmacists, pharmacy technician technician, resident, and intern practice, prioritizes and organizes workflow, and checks accuracy of pharmacy technician activities. Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems. Verifies and enters medication orders, proactively communicating with medical staff to resolve problems. Provides accurate, safe, and timely medication therapy, and promotes cost-effective drug therapy. Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults. Participates in quality improvement initiatives to enhance pharmacy services and patient outcomes. Teaches and mentors pharmacy students, providing guidance and support for their professional development. Other duties as assigned.Knowledge, Skills, and Abilities:

• Ability to train and mentor all pharmacy staff [Required]
• Ability to participate in clinical activities and documentation such as IV to PO, renal dosing and pharmacokinetics [Required]
• Initiative to keep abreast of developments in the provision of pharmacy services [Required]
• Initiative to keep abreast of new information/products/developments in the field of pharmacy [Required]
• Proficiency in general hospital pharmacy operations and regulatory compliance [Required]
• Proficiency with hospital computer systems, pharmacy technology, and pharmacy automation [Required]
• Proficiency in hospital pharmacy practice, patient-centered care, advanced pharmacy practice, medication therapy management, and evidence-based medicine [Required]
• Must be able to read, write and speak conversational English [Required]
• Intermediate computer skills and knowledge of Microsoft Office [Required]
• Supervises Clinical Programs and Pharmacy Education and Training [Preferred]

Education:

• Bachelor’s graduate of an accredited pharmacy school [Required]
• Doctorate doctorate in pharmacy [Preferred]

Field of Study:

• N/A

Work Experience:

• 1+ pharmacy supervisor experience [Preferred]
• 3+ hospital pharmacy experience [Required]

Additional Information:

•Pharmacy Residency Training [Preferred]
• Applicable State Pharmacy License Upon Hire Required

• Consultant Pharmacist License as applicable per State Board of Pharmacy Upon Hire Preferred

• Board Certification Upon Hire Preferred

Licenses and Certifications:
