The RN Transfer Center Supervisor is responsible for coordinating complex, high-acuity patient transfers, admissions, and inter-hospital patient movement. This position uses strong clinical judgment to determine the appropriate department, patient care unit, level of care, trauma center, pediatric hospital, or other receiving facility based on the patient’s diagnosis, severity of illness, and intensity of services required.
Role Summary
The RN Transfer Center Supervisor is a Full-Time role supporting Nights and is responsible for coordinating complex, high-acuity patient transfers, admissions, and inter-hospital patient movement. This position uses clinical judgment to determine the appropriate department, unit, level of care, trauma center, or pediatric hospital based on the patient’s diagnosis, severity of illness, and intensity of services required.
The role collaborates closely with physicians, nursing leadership, Social Work, Case Management, Emergency Department staff, Surgical Services, regional hospitals, and other departments to ensure patients are transferred safely and efficiently to the most appropriate destination. The Transfer Center Supervisor also supports timely bed placement, facilitates communication among care teams, and maintains accurate transfer and admission documentation to ensure patients are safely and efficiently connected to the right level of care.
What You’ll Do

As the Transfer Center Supervisor, you will coordinate the placement and transfer of high-acuity patients who require specialized services, critical care, trauma care, or pediatric care.

Oversee and support the work of the Staffing Coordinator and Patient Registrar, including completing annual evaluations.

Evaluate patient needs and determine the most appropriate department, patient care unit, trauma center, pediatric hospital, or receiving facility.

Partner with Social Work, Case Management, physicians, nursing leaders, Emergency Department staff, Surgical Services, and other departments to coordinate safe and appropriate patient movement.

Assure efficient placement for direct admissions, Emergency Department admissions, inter-hospital transfers, post-operative patients, and intrahospital transfers.

Facilitate admission and transfer processes related to the Transfer Center, Emergency Department, and Surgical Services.

Communicate with physicians and outlying facilities regarding patient condition, placement needs, transfer requirements, and available resources.

Escalate complex or unresolved placement concerns through the established chain of command.

Verify that patient information is obtained, communicated, and documented accurately and promptly.

Maintain complete and accurate records related to patient transfers, admissions, consultations, and placement decisions.

Build respectful relationships with internal and external customers by listening, understanding their needs, and providing effective solutions.
Key Responsibilities

Ensures efficient patient placement for direct admissions, Emergency Department admissions, transfers, post-operative patients, and intrahospital transfers.

Coordinates patient placement according to established protocols while securing timely and accurate patient information.

Strong clinical judgment and decision-making abilities.

Ability to assess the needs of high-acuity patients and coordinate appropriate placement.

Ability to collaborate effectively with Social Work, Case Management, physicians, nursing leadership, and multiple hospital departments.

Strong organizational and problem-solving skills.

Ability to manage multiple transfers, admissions, and placement needs in a fast-paced environment.

Facilitates admission processes related to the Transfer Center, Emergency Department, and Surgical Services.

Proficiency in nursing computer applications and Microsoft Office products, including Excel and Word.

Excellent written and verbal communication skills.

Maintains accurate records and documentation related to patient transfers and admissions.
Qualifications
-
Bachelor of Science in Nursing required.
-
Two or more years of nursing experience required.
-
Two or more years of leadership experience required.
-
Critical Care or Emergency Department experience preferred.
-
Case Management experience preferred.
Licenses and Certifications
-
Registered Nurse license required.
-
Basic Life Support—CPR certification required.
