Job Description:

Conducts comprehensive assessments of patient and donor psychosocial, cultural, and spiritual needs through interviews, observations, and clinical data. Evaluates family relationships, support systems, mental health history, personality traits, and coping skills to inform care plans. Collaborates with the transplant team to develop and implement individualized care plans that address the holistic needs of patients and donors. Provides counseling and support to patients and donors, facilitating decision-making and addressing concerns related to the transplant process. Participates in multidisciplinary rounds and transplant team meetings to discuss patient and donor progress and care plans. Refers patients and donors for additional evaluations, such as neuropsychological assessments, as needed. Engages in discharge planning, coordinating healthcare and community services to support patients and donors post-transplant. Educates patients and donors on the financial aspects of transplantation, including insurance coverage and available resources. Advocates for patients and donors, ensuring their needs and preferences are respected and addressed. Provides individual or group therapy sessions to support patients and donors in coping with the transplant process. Other duties as assigned.Knowledge, Skills, and Abilities:

Pay Range:

$62,108.80 – $115,544.00
